Health Insurance Claim Deduction: Room Rent, Customary Charges and Complaints

Health Insurance Claim Deduction: Room Rent, Customary Charges and Complaints

Health insurance claim deduction? Get the complete settlement breakdown first

The hospital billed ₹2 lakh, but your insurer approved only ₹1.45 lakh. You paid the difference and now want to challenge it. Start by identifying each deduction, the applicable policy clause and the calculation. A focused complaint about a particular expense is more useful than a general demand to pay the entire hospital bill.

Not every deduction is incorrect. However, an unexplained reduction deserves a written response. Room limits, proportionate deductions, co-pay, deductibles, non-payables and reasonable-and-customary charges are different issues. This guide explains what to ask for and how to escalate without promising a guaranteed recovery.

Five reasons the approved amount may be lower

Deduction Question to ask
Room-rent limit Was the room within your eligible category or daily limit?
Proportionate deduction Which associated medical expenses were reduced because of the room category?
Co-pay or deductible What share or amount did your policy allocate to you?
Non-payable or sub-limit Which exclusion, treatment limit or add-on applies?
Reasonable and customary What comparison, rate and clinical circumstances support the reduction?

Several grounds can appear in the same bill. A pharmacy expense rejected under an exclusion raises a different issue from a pharmacy expense reduced using a room-category ratio. Separate the heads instead of treating the total difference as one undifferentiated dispute.

A TPA may assist with processing, but the insurer issued the policy. Seek clarification from the TPA while also lodging the formal grievance with the insurer’s published grievance officer. A verbal comment at the hospital desk is not a substitute for a written decision.

What reasonable and customary charges mean

The term generally refers to a provider’s standard charges and prevailing local charges for comparable services, taking the illness or injury into account. An insurer’s published wording illustrates this definition. Read your own contract: another product’s wording does not govern your policy.

If surgeon or operating-theatre fees were reduced as excessive, ask which clause was applied, what rate was accepted, what treatment/provider comparison was used and how the patient’s clinical complexity was considered. Frame these as requests for explanation and evidence.

Ask the hospital for an itemised invoice, applicable tariff, package breakdown and explanation of the disputed charge. Where medical necessity is questioned, request a relevant explanation from the treating doctor. Do not ask anyone to inflate a bill or alter the treatment history.

There is no sound universal shortcut saying that the absence of one government tariff database automatically invalidates every reduction. The issue is whether the policy and applicable rules support the assessment with suitable reasons and evidence. A high charge is not automatically unreasonable; a hospital invoice is not automatically fully covered either.

Check room-rent proportionate deductions carefully

Some policies permit a room-category upgrade to affect both the room difference and certain associated medical expenses. A hospital may vary professional, procedure or theatre fees with the chosen room. Check the actual schedule and wording before accepting the resulting calculation.

FACT: IRDAI’s proportionate-deduction instructions require defined associated expenses and attention to differential billing. They exclude pharmacy/consumables, implants/medical devices and diagnostics from that definition and prohibit room-category proportionate deductions on ICU charges. This does not make every such expense payable despite separate exclusions or limits.

See the IRDAI circular listing for 11 June 2020, IRDAI/HLT/REG/CIR/151/06/2020. Review your policy version and subsequent applicable requirements together.

Illustration only: if eligible room rent is ₹4,000 and actual rent is ₹6,000, a relevant policy may use a ratio of about 66.67% for eligible associated heads. This is not a universal formula for reducing the entire bill. Identify the heads, tariff practices and any separate limits.

If the hospital charges the same fee for a procedure across room categories, its written tariff or confirmation can support the objection. If the eligible room was unavailable, preserve availability information. That fact alone does not guarantee that the contractual limit disappears.

Cashless treatment does not guarantee full coverage

Cashless is a payment arrangement, not a promise that every billed expense is insured. Initial pre-authorisation, enhancement and final discharge approval are different stages. When expenses rise, ask the hospital insurance desk to submit an updated request and preserve the insurer’s response.

Reconcile the final bill, authorised amount, hospital discount, insurer payment and patient payment. If a negotiated hospital-insurer reduction is being charged to you, ask both sides for the contractual basis. Do not assume every difference belongs to the patient without clarification.

If you paid a disputed amount, retain the receipt and payment reference. A concise written record of your objection may help document the dispute, but writing “under protest” does not guarantee reimbursement. Record the amount and grievance separately.

Make treatment and medically appropriate discharge decisions with the treating team. Do not alter care to prolong a claim complaint. Financial assessment and patient safety require separate attention.

Escalation: insurer, Bima Bharosa and Ombudsman

  1. Insurer grievance officer: Submit policy/claim references, disputed heads, the relief requested and supporting evidence. Keep acknowledgement and a complaint number.
  2. Bima Bharosa: Use the IRDAI portal to register/track an unresolved or unsatisfactorily answered grievance. It is a monitoring mechanism, not an automatic refund decree.
  3. Insurance Ombudsman: After a prior representation to the insurer, assess eligibility if the response is unsatisfactory or no response is received for one month. The official complaint portal sets out conditions.

Bima Bharosa’s homepage says complaints will be attended to within 14 days. This does not promise claim repayment in 14 days or remove the Ombudsman’s separate eligibility requirements. Registration, a response and adjudication are different events.

The published Ombudsman conditions include a ₹50 lakh monetary ceiling and generally a one-year filing period linked to rejection or the expiry of one month without a response. Do not treat the ceiling simply as a cutoff based on your total sum insured; check the actual loss/relief and current form requirements.

The CIO FAQ confirms that filing is free and does not require going through a lawyer. The same subject already pending or decided in court, a consumer forum or arbitration raises an admissibility restriction. Do not submit identical proceedings everywhere at once.

State the disputed amount rather than claiming the entire hospital bill when only certain heads are challenged. Requesting interest or compensation does not establish entitlement. No reliable guarantee supports a 90% success promise or a fixed 8–9% interest outcome.

Evidence checklist and complaint template

  • Policy schedule, full wording, Customer Information Sheet and relevant endorsements/add-ons.
  • Itemised final invoice, payment receipts, discharge summary and relevant medical records.
  • Deduction/settlement sheet, insurer/TPA communication and cashless approvals.
  • Hospital tariff, package breakdown and a focused doctor explanation where relevant.
  • Initial grievance, acknowledgement, response and a short dated chronology.

Prepare a comparison sheet: billed amount, accepted amount, stated reason and your specific objection. If a revised hospital invoice corrects a billing error, clearly identify the revision; do not quietly substitute conflicting documents.

Keep sensitive medical documents within official channels and limit them to what the complaint needs. Do not publish patient records in public messaging groups. Maintain a complete copy of each submission so you can answer requests consistently.

Subject: Review of ₹[amount] deduction — health claim [reference]

Under policy [reference], ₹[deduction] was reduced from [expense head] for [stated reason]. Please provide the applicable policy clause, accepted rate/benchmark and item-wise calculation. My objection is supported by [tariff/room eligibility/add-on/medical clarification], attached. Please review each disputed head, give a reasoned written decision and repay any amount found to have been incorrectly deducted. Advice

Keep hospital and insurer responses in separate folders. Read any full-and-final release before signing a settlement voucher. Never use false invoices or an altered medical history. Do not share OTPs or pay a QR request to receive a refund. Bima Bharosa explicitly warns that it does not ask complainants for payment.

Before future planned admission, ask about room eligibility and tariff practices, while recognising that emergency circumstances may limit choices. Keep written answers and check add-on coverage rather than relying only on a salesperson’s assurance.

FAQ

Are reasonable-and-customary reductions always wrong?

No. Check the definition, grounds and evidence. Challenge unclear or unsuitable comparisons specifically.

Can a higher room category reduce every expense?

Do not assume one ratio applies throughout. Check associated heads, differential billing and separate policy limits.

Does protection from a room-related deduction cover all consumables?

No. Separate non-payable conditions or add-on coverage can still matter.

Is emailing the TPA enough?

Also send a formal grievance to the insurer’s published grievance officer and keep acknowledgement.

Does the Ombudsman guarantee refund and interest?

No. Relief depends on the contract, evidence and decision. Filing is not a guaranteed win.
