What the Commission ordered on 2 October
Where SIR is complete, the Election Commission ordered a special drive. Reports counted 20 states and Union Territories. Booth level officers will compare the old and new rolls, visit people whose names disappeared, and help eligible voters file Form 6. Each BLO was told to cover 20 to 25 such voters a week, so the drive finishes in about a month.
During SIR, Form 6 carried an extra declaration. The Commission said that where revision is over, the ordinary statutory Form 6 will do. The ERO, not the BLO, takes the final call under the 1950 Representation of the People Act and the 1960 registration rules.
What Form 6 is for
Form 6 is for inclusion. First-time voters use it too. You must be 18, at an address in that booth. In many states a name deleted in SIR does not return by itself. It comes back through Form 6. Shifting and correction forms are different. Check the state CEO page before you guess the form number.
The online door is voters.eci.gov.in. An EPIC number helps. If you do not have one, search by name, relative and age. Photo and address proof can be asked. Do not trust a WhatsApp forward that says Aadhaar is compulsory or banned. Read the form instruction.
Why a name may have gone
The Commission cites death, a shift, duplicates, or a failed map in SIR. The opposition says poor, tenant, Dalit, Adivasi and Muslim voters were cut more often. Both can be true in parts. Removing the dead is cleanup. Cutting a living voter without notice is a complaint. The complaint route is the form and the ERO.

What to keep at home
Old voter card, address proof, a photo, and a screenshot of the pre-SIR roll if you have it. Take a receipt from the BLO. Write the application number. If the ERO refuses, ask for the reason in writing. Appeal windows are short.
